Registered Nurse (RN) Case Manager
Coordinates and oversees patient care in the home setting, develops individualized care plans, performs skilled nursing visits, and ensures compliance with medical protocols and agency policies.
Licensed Practical Nurse (LPN)
Provides nursing care under the supervision of an RN, including administering medications, monitoring vital signs, and supporting patients with their care plans in the home environment.
Physical Therapist (PT)
Assesses and treats patients to improve mobility, strength, and physical function. Develops therapy plans to restore independence and prevent further physical limitations.
Occupational Therapist (OT)
Helps patients develop or regain skills needed for daily living and self-care. Evaluates functional abilities and designs personalized therapy programs for home-based rehabilitation.
